Child and Family Support Team The Child and Family Support Team is comprised of a Certified School Nurse and a Certified School Social Worker. The Purpose of the School Nurse/School Social Worker Team is to work together to identify children who are failing, at-risk of academic failure, and/or have Out of Home Placement. The goal of the Team is to assist families in decreasing health and socio-economic barriers. The process involves referrals for Screenings, Resources, and Child and Family Team Meetings with families to make a plan to increase student success.
